Please use this form to enter your bank account details for payment of your fundraiser profits. You should provide your bank account number in the field provided and upload a scanned copy of the official deposit slip or the top section of an official bank account statement below. The image must show the Bank Account Name and Bank Account Number. We cannot pay funds to a personal account – it must be to an official School or Club account.

After completing this form an email from [email protected] will be sent to the email address you provide – this must be the official email of the person who is authorised to provide these bank account details. 

YOU MUST REPLY TO THE CONFIRMATION EMAIL AS THE FINAL STEP OF THIS PROCESS. IF YOU DO NOT SEE THIS EMAIL WITHIN 5 MINUTES OF COMPLETING THE FORM PLEASE CHECK YOUR JUNK MAIL.
